HOW TO EMBED A LIVE FORM

Create your form in Google Docs: Forms
Go to > More Actions > select Embed. An html code will appear. Copy this.
Go back to your wiki > Edit this page
Select > Embed widget > paste code into field.
Save and go to wiki page to see the actual live form embedded on your page.
